29 CFR 1926.150(c)(1)(viii): Portable fire extinguishers were not inspected periodically in accordance with maintenance and use of portable fire extinguishers, N.F.P.A. No. 10A-1970: On or about July 19, 2012, employee operating a Roofing Tar Kettle where flammable gas was being used on the jobsite, was provided with a discharged fire extinguisher. The employee was exposed to fire and/or burn hazards.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. On or about July 17, 2012, employees performing roofing activities on a commercial building were not protected from falling more than (13) feet to the ground below.
