On the North and East sides of the building, the mud sill plates for the outriggers on the Hydro-Mobile scaffold were placed on unlevel, unstable ground, thereby exposing employees to a fall or crushed-by hazard.

29 CFR 1926.451(f)(7):     Scaffolds shall be erected, moved, dismantled, or altered only under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration. Such activities shall be performed only by experienced and trained employees selected for such work by the competent person.    a. At the worksite, employees were laying block and mortar from the working platforms of ten M2 Hydro-Mobile scaffolds that were erected without the supervision or direction of a competent person.

29 CFR 1926.50(g):     Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use.
