The endpoints at each upright were more than 48 inches (1.3 m) apart. a) Morning Star Fellowship Church - On or about July 16, 2012, an employee is working from an outrigger at a height of no less than 12 feet, using the crossbracing as fall protection.

JOHN MINDLER PLASTERING, WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 312973225, CITATION NUMBER 1, ITEM NUMBER 2b, AND WAS AFFIRMED AS A FINAL ORDER ON JULY 6, 2009, WITH RESPECT TO A WORKPLACE LOCATED AT 1446 W. BROAD STREET, QUAKERTOWN, PA.
