29 CFR 1926.602(c)(1)(ii): Modifications or additions affecting the safe operation of the equipment were made without the manufacturer's written approval:  Note:  If such modifications or changes were made, the capacity, operation, and maintenance instruction plates, tags, or decals should have been changed accordingly:    a)  Trico Construction, LLC at 2217 Wagon Way, Louisville, CO 80027:  On or before July 14, 2012, the employer did not ensure that the attachment to the Gradall JLG model 534D9-45, consisting of an extension to handle trusses, had received the forklift manufacturer's approval.  The attachment extended the rigging connection point out by about 15', and the modified load capacity charts were not posted or available.  The attachment was manufactured by RLS Specialty Fabrication, serial number 15106791.  This lattice boom style attachment was used to lift a bundle of 3 roof trusses and extend them above the second floor top plate for the length of the house.  An employee working on an interior second floor top plate had to duck under the load as it passed by.  The forklift operator did not know the weight of the roof trusses, nor the capacity of the forklift at this extended height and reach using this attachment.  Condition exposed this employee to the hazard of getting struck by falling loads.
