29 CFR 1926.453(b)(2)(v): A body belt was not worn with a lanyard attached to the boom or basket while working from an aerial lift: a) Walcher & Sons, Inc. on or about July 20, 2012, at the address of 155 Allen Rd. Bellevue, OH: The employer failed to ensure employees working from two different aerial lifts were adequately protected from falls. Employees working from the lifts were not clipped off to the basket while the platform was elevated. The employees were exposed to falls up to 18 feet to the ground below.
