29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.    (a)	Rock On Framing, Inc., 575 24  Rd., Grand Junction, CO 81501:  On and before July 19, 2012 the employer did not provide hard hats to employees who were working below an aerial lift.    Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.453(b)(2)(v):  A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    (a)	Rock On Framing, Inc., 575 24  Rd., Grand Junction, CO 801501:  On or before August 28, 2009, an employee was attaching siding from the basket of a Genie S-60 aerial lift and was not wearing fall protection. This hazard exposed the employee to a fall of approximately 10 feet.    Abatement Note: A personal fall arrest system is recommended with lanyard attached to the boom or basket when working from an aerial lift. As of January 1, 1998, Subpart M of the 29 CFR 1926 part 1926.502(d) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).    Abatement Note:  Abatement certification is not required for this item (see enclosed Abatement-Certification Letter").

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    (a)	Rock On Framing, Inc., 575 24  Rd., Grand Junction, CO 801501:  On or before July 18, 2012, an employee of the roof was not wearing fall protection while attaching siding to the building. This exposed the employee to a fall of approximately 18 feet.     Abatement Note:  Abatement certification is not required for this item (see enclosed Abatement-Certification Letter").
