Cotton St, Portageville, on 7-22-12, where employees were lifted to work on a 7:12 pitch roof in a man basket, lanyards were unattached by two of three employees because of insufficient lanyard length to access the work (removing roof panels; eave of lowest roof was over 12 ft.).
