29 CFR 1910.134(e)(1):  The employer did not provide a medical evaluation to determine the employees ability to use a respirator, before the employees were fit tested or required to use a respirator in the workplace:    (a)	Fort Collins Monument and Stone, LLP, 824 E. Lincoln Ave., Ft. Collins, CO 80524: On and before July 17, 2012, the employer did not ensure that all employees who wear a tight fitting half mask respirator had received medical evaluations.     Abatement Note:  Abatement certification is required for this item (See enclosed "Certification of Corrective Action Worksheet").

Lincoln Ave., Ft. Collins, CO 80524: On and before July 17, 2012, the employer did not ensure that employees they required to wear 3M tight-fitting half mask respirators passed a fit test on an annual basis. When asked where the sheets were located the employees did not know.
