29 CFR 1926.451(b)(1):     Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows:    a.  At the site, located on the side of the building, employees were grouting stone on the outside of a masonry wall from a tubular weld frame scaffold.  The scaffold that the employees were working from only had one plank for the employees to perform their work, thereby exposing the employees to a fall hazard of a max height of 13-1/2 feet.       b.  At the site, located on the front of the building, employees were grouting stone on the outside of a masonry wall from tubular weld frame scaffold.  The scaffold that the employees were working from only had one plank for the employees to perform their work, thereby exposing the employees to a fall hazard of a max height of 9-1/2 feet.

29 CFR 1926.451(c)(2):     Supported scaffold poles, legs, posts, frames, and uprights shall bear on base plates and mud sills or other adequate firm foundation.    a.  At the site, located on the side of the building, employees were grouting stone on the outside of a masonry wall from a tubular weld frame scaffold.  The scaffold that the employees were working from had two pole legs that were positioned on the ground that did not have any mudsills, thereby exposing the employees to a max fall hazard of 13-1/2 feet if a fall were to occur due to the instability of the scaffold.      b.  At the site, located on the front of the building, employees were grouting stone on the outside of a masonry wall from a tubular weld frame scaffold.  The scaffold that the employees were working from had two pole legs without base plates and one pole leg without a mudsill, thereby exposing the employees to a fall hazard of 9-1/2 feet if a fall were to occur due to the instability of the scaffold.

29 CFR 1926.451(e)(1):     When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.    b.  At the site, located on the side of the building, an employee was hanging plastic on the top of a tubular weld frame scaffold's crossbracing.  The employee would climb from the crossbracing to the work platform, thereby exposing the employee to a fall hazard of 9-1/2 feet.

29 CFR 1926.451(g)(1):     Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level.     a.  At the site, employees were attaching plastic to the frame of a fabricated frame scaffold for protection from the rain without any means of protection from a fall hazard, thereby exposing the employees to fall hazard of 13-1/2 feet.
