29 CFR 1926.451(b)(5)(i): The end of a platform 10 feet or less in length extended over its support more than 12 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees and/or materials without tipping, or has guardrails which block employee access to the cantilivered end:          (a) Jobsite:  On or about July 19, 2012, employees were working from a tubular welded frame scaffolding and the wooden planking (that was less than 10 feet) extended over its end supports by as much as 2 1/2 feet.  The wooden planks were not secured from tipping.  The fall hazard to the employees was approxiamtely 30 feet.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:        (a) Jobsite:  On or about July 19, 2012, two employees were working from a tubular welded frame scaffold and they were not protected from falling.  The fall hazard to the two employees was approxiamtley 30 feet.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:            (a) Jobsite:  On or about July 19, 2012, two employees were not protected from falling from the roof as they left the tubular welded frame scaffold to go onto the roof and then walked up the roof to go back onto the tubular welded frame scaffolding.
