29 CFR 1926.453(b)(2)(iv):  Employee(s) were not standing firmly on the floor of the basket and were climbing on the edge of the basket.     a) Interior of building - Employees were exposed to a fall hazard of approximately 17-feet to the lower level while working from an aerial lift.  The employees were standing on the railing, and entering and exiting the lift by going over the railing.

29 CFR 1926.453(b)(2)(v):  A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    a) Interior of building - Employees installing metal decking bracing were exposed to a lower level fall hazard of approximately 17-feet by not using body belts or full body harnesses and lanyards while elevated in an aerial lifting device.

29 CFR 1926.760(a)(1):  Each employee engaged in a steel erection activity who was on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.    a) Roof joist - Employees installing metal deck bracing were exposed to a lower level fall hazard of approximately 22-feet by not using any means of approved fall protection equipment, to include: guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.
