29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables used for temporary wiring were not protected from damage: a) On or about June 20, 2012 and at times prior Adolfson & Peterson Construction, as the controlling contractor did not ensure that workers were protected from electrical hazards. Extension cords used at the WPA Gym were not protected from damage. The temporary panel box was located so that the cords had to run across the access area where vehicles drove across them. The cords were also run over by forklifts used by the masonry company in the work area.

29 CFR 1926.502(b)(13): When guardrail systems were used around holes which were used as access (such as ladderways), they were not provided with a gate, or were not so offset that a person could not walk directly into the hole: a) On or about June 20, 2012 and at times prior Adolfson & Peterson Construction, as the controlling contractor, did not ensure that workers were protected from fall hazards. The scaffold stairway used to access the ceiling area did not have guardrails installed to protect the stairway opening.

The distance from the top of the scaffold stairway to the work platform was over 19 inches.
