A) 112 Woodward Avenue, Rutherford, NJ: Employees were exposed to fall hazards of approximately 22 feet while working on a fabricated framed scaffold that was not protected by a guard rail system, a personal fall arrest system or other means of fall protection, on or about July 10th 2012.
