29 CFR 1910.22(a)(1):     All places of employment, passageways, storerooms, and service rooms shall be kept clean and orderly and in a sanitary condition.    On or about 7/13/2012 , employees were exposed to a fire and health hazards as a result of poor housekeeping from the presence of accumulated combustible materials and debris such as but not limited to: empty corrugated boxes, waste paper, plastic bags, beverage containers and auto parts at a facility site located at 2102 South Federal Highway, Delray Beach, FL. 33483.

29 CFR 1910.141(a)(4)(ii):     All sweepings, solid or liquid wastes, refuse, and garbage shall be removed in such a manner as to avoid creating a menace to health and as often as necessary or appropriate to maintain the place of employment in a sanitary condition.    On or about 7/13/2012, employees were exposed to health hazards from the presence accumulated refuse and garbage in containers that remained inside the facility overnight that attracted rodents at a facility located at 2102 South Federal Highway, Delray Beach, FL. 33483.

29 CFR 1910.141(a)(5):     Vermin control. Every enclosed workplace shall be so constructed, equipped, and maintained, so far as reasonably practicable, as to prevent the entrance or harborage of rodents, insects, and other vermin. A continuing and effective extermination program shall be instituted where their presence is detected.    On or about 7/13/2012 , employees were exposed to health hazards from the presence of rodents and rodent droppings on floors and shelves at a facility that lacked an effective extermination program and permitted the entrance of rodents at a facility located at 2102 South Federal Highway, Delray Beach, FL. 33483.

29 CFR 1910.305(b)(2)(i):     All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.    On or about 7/13/2012 , employees were exposed to an electrical shock hazard while working in proximity to energized 110 VAC receptacle outlets and junction boxes that were not provided with  covers at a facility located at 2102 South Federal Highway, Delray Beach, FL. 33483.       Three Instances:  (a) Single receptacle in the Technicians locker room.  (b) Junction box in the Technicians locker room.  (c) Duplex receptacle in the parts counter area.
