29 CFR 1910.24(b): Fixed stairs were not provided for access from one structure level to another where operations necessitated regular travel between levels, and for access to operating platforms at any equipment which requires attention routinely during operations: (a) Fixed stairs were not provided for accessing the tops of injection molding machines. Mold Setters were climbing the plattens in injection molding machines to access the top of the mold where the overhead lift would be attached during mold change operations. This occured at injection molding machine #16 but was not limited to injection molding machine #16. Abatement certification is required for this item. Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose. 29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides: (a) Employees removing or installing large molds while standing on injection molding machines were exposed to approximately a seven (7) foot fall hazard with no guard railings or the equivalent fall protection. This happens, but is not limited to, when mold setters install/remove the Prada Super Barrel mold on injection mold machine #16. Abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.179(k)(1)(i): Prior to initial use, new or altered crane(s) were not operationally tested to insure compliance with 29 CFR 1910.179: (a) On or about April 2003, an OMi overhead top running under hung bridge crane, serial number 12724, was partially dismantled and the trolley was removed and transferred to OMi Crane Systems, Inc. for alterations performed under warranty. When the altered crane trolley was reinstalled, it was not tested to insure safety devices were operating correctly. Employees were exposed to the hazards created by crane failure. On or about July 20, 2012, OMi overhead top running under hung bridge crane, #12724, failed resulting in injury to one employee and the death of one employee. Abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.179(l)(1): A preventive maintenance program based on the crane manufacturer's recommendations was not established: (a) There are twelve (12) cranes in the facility. Preventative maintenance programs based on the manufacturers' recommendations were not established. An improperly maintained crane could become inoperative or fail exposing employees to hazards created by falling materials. On or about July 20, 2012, OMi overhead top running under hung bridge crane, #12724, failed resulting in injury to one employee and the death of one employee. 29 CFR 1910.179(l)(3)(i): Unsafe condition(s) disclosed during the frequent or periodic inspection(s) required by 29 CFR 1910.179(j) were not corrected before operation of the crane(s) was resumed: (a) The up button cover on the pendent control for the OMi overhead top running under hung bridge crane, Serial Number 12724, was torn off. The pendent button cover was not inspected and replaced prior to being put back into service. The cover was torn off prior to the accident that occurred July 20, 2012. (b) OMi overhead bridge crane, Serial Number 12724, had been reported as having North/South travel disturbances or moments of incorrect operation. The crane had the problem intermittently as reported May 3, 2012, in the Pre-Operation Crane Inspection by employees. Maintenance placed it back in service. The problem was again reported June 27, 2012. Again maintenance put the crane back in service. The company took no further action to determine what malfunction the equipment was having. Crane maintenance and repair contractor services to evaluate and remedy the problem were not hired. On or about July 20, 2012, OMi overhead top running under hung bridge crane, #12724, failed resulting in injury to one employee and the death of one employee. Abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
