29 CFR 1926.451(c)(2)(v):     The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:    a) Northwest Corner of Structure:  On or about June 20, 2012, employees were tasked to work on the platform of a makeshift man-lift to frame the soffit and fascia along the upper and lower-level roof overhang.  The plywood-platform that the employees used as a working platform was not attached/secured to the forks, exposing employees to a 7 foot, 3 inch-fall hazard from first level and no less than a 20 foot fall hazard from the second level along the roof's edge.

29 CFR 1926.501(b)(1):     Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a) First Level Overhang:  On or about June 20, 2012, employees were using the roof of the first level overhang as a working platform to frame the soffit and fascia along the overhang's edge.  The workers were not wearing fall protection, exposing employees to approximately an 11 feet fall hazard.    b) Interior second Level/Mezzanine Opening  On or about June 20, 2012, employees worked on and or retrieved water from the Second Level of the facility/structure, where the mezzanine opening was not guarded, exposing employees to a 11 feet, 2 inch- fall hazard.
