On or about 07/16/12, at the site of 50-24 216th Street. Bayside, NY Employees working in an excavation were in danger of head injury from falling objects such as, but not limited to foundation forms from an excavator machine an operator swing over the employees. Protective helmets were not worn. ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19

ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.
