On July 20, 2012, at the job site on the Southwest corner of the building where employees had an extension cord set that had a house hold 110 plug wired and taped, exposing employees to electrical shock.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On July 20, 2012, at the job site on the Southwest corner of the building employees were working on the roof and where not wearing fall protection. The employees were exposed to falls greater than 6 foot. STONE GATE ROOFING LLC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 1926.501(B)(11), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 315910687, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON MARCH 22, 2012, WITH RESPECT TO A WORKPLACE LOCATED AT 2101 GREENHOUSE ROAD, KATY, TEXAS 77094.
