29 CFR 1926.451(b)(1)(i):   Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.    a.   On or about July 9, 2012, on the north side of the building, the scaffold platform was not fully planked, exposing employees to a 10 feet 10 inches fall hazard.

29 CFR 1926.451(c)(2):  The supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:    a.  On or about July 9, 2012, on the north side of the building, the scaffold poles, legs, and frames were bearing directly on wood or concrete blocks and not base plates, exposing the employees to a fall hazard of 10 feet 10 inches.    b.  On or about July 9, 2012, on the north side of the building, the scaffold poles, legs, and frames were bearing directly on wood or concrete blocks and not base plates, exposing the employees to a fall hazard of 6 feet 8 inches.

On or about July 9, 2012, on the east side of the building, the scaffold was supported by concrete blocks, exposing employees to a fall hazard of 6 feet 8 inches.

29 CFR 1926.451(e)(1):  When scaffold platforms were more than 2 feet (0.6m) above or below the point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used:    a.   On or about July 9, 2012, on the north side of the building, a safe means of access was not provided to the second level working platfom, exposing employees to a 10 feet 10 inches fall hazard.    b.   On or about July 9, 2012, on the east side of the building, a safe means of access was not provided to the first level working platform, exposing employees to a 6 feet 8 inches fall hazard.

On or about July 9, 2012, on the north side of the building, the scaffold did not have a guardail on all open sides and ends, exposing emplyees to a 10 feet 10 inches fall hazard.
