On or about July 16, 2012, machine guarding was not provided to protect the operator and other employees in the shop area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. The horizontal band saw, used to cut metal, was not guarded to prevent employee contact with the moving blade.
