29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or about July 19, 2012, the employer failed to ensure that a lockout/tagout (LOTO) program was developed for the following equipment: 1) Cutter in the label shop; employee(s) perform LOTO on the cutter if the machine breaks or needs to be oiled. The machine has two power sources, air and electric. 2) Heidelberg plate machine in the print shop/plate processing room; every 3 or 4 months employee(s) change chemicals and filters on the plate machine and remove jams when they occur. The Heidelberg plate machine has two power sources, air and electric.

29 CFR 1910.305(a)(2)(i)(B): Temporary electrical power and lighting installations of 600 volts, nominal, or less was used for a period exceeding 90 days. On or about July 19, 2012, the employer failed to ensure that permanent electrical outlets were installed in the label shop. Extension cords were being used as permanent wiring for the Jogger (scale) and conveyor for recyclable paper.
