29 CFR 1926.651(b)(2): Utility companies or owners were not contacted within established or customary local response times, advised of the proposed work, and asked to establish the location of the utility underground installations prior to the start of an actual excavation: Building parking/road concrete areas, west side of building, west center area of worksite: On 07/20/2012, the employer did not contact the utility company to request identification of any workplace underground installations prior to starting concrete slab boring activities. Employees engaged in the boring activities were exposed to thermal burns from contact with flammable natural gas leaking from the damaged underground utility installation.
