29 CFR 1926.95(a):  Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    a) Exterior Rear of Structure - A laborer was observed striking a crow bar with a 2 pound hammer while removing existing exterior brick, and not wearing eye protection, on or about 07/19/12.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a) Second Floor Rear of Structure - A laborer was observed standing on an open sided floor while dropping demolition debris into the bed of a state body truck, and exposed to a 16 ft, 6 inch fall hazard to an asphalt alleyway below, on or about 07/19/12.
