29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:. Location: 38 Forest Place, Brooklyn, NY. a) Employer failed to provide a personal fall arrest system for employees installing Spanish tiles on the roof of a 3 story building. The employees were exposed to a potential fall of approximately 30 ft to the concrete floor below; on or about 7/17/12.

29 CFR 1926.503(b)(1): When the employer had reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer shall retrain each such employee. Location: 38 Forest Place, Brooklyn, NY. a) Employees did not use personal fall arrest systems when installing Spanish tiles on the roof of a 3 story building. The employees were exposed to a potential fall of approximately 30 ft to the concrete floor below. Fall protection re-training is required; on or about 7/17/12.
