29 CFR 1910.178(l) (3) (ii) (H): Powered industrial truck operators did not receive initial training on workplace-related topics including closed environments and other areas where insufficient ventilation or poor vehicle maintenance could cause a buildup of carbon monoxide or diesel exhaust. On or about July 10, 2012, the employer did not provide employees with training on the hazards of operating propane and diesel powered industrial trucks in an enclosed area, exposing the employees to inhalation hazards. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the employees are properly trained on the effects of powered industrial truck exhaust.

On or about July 10, 2012, the employer did not ensure that adequate ventilation was in place to prevent employee exposure to carbon monoxide where propane, diesel and gas, powered industrial trucks were operating, exposing employees to the hazard of inhaling noxious air contaminants. (Measurements of carbon monoxide reached up to 300 ppm for a period of one hour.) Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that adequate ventilation is in place where powered industrial trucks will be operating.
