29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    On or about July 24, 2012, on the Northside of the building; employees were observed removing an awning sign from the edge of a roof top approximately 15 feet high without fall protection. This condition exposed the employees to a fall hazard.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    On or about July 24, 2012, on the Eastside of the building; an employee was observed accessing a roof top approximately 15 feet high with the use of an extension ladder where the side rails of the ladder did not extend at least 3 feet above the upper landing surface. This condition exposed the employee to a fall hazard.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step:    On or about July 24, 2012, outside the entrance to the furniture store; an employee was observed standing on the top of a 10 foot step ladder while removing an awning sign. This condition exposed the employee to a fall hazard.
