29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    (a)  The employer failed to provide employees with a training program for fall hazards which provided them with procedures to eliminate those hazards.  This exposed employees to fall hazard when they were cleaning the exterior portion of the windows.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.    (a) The employer failed to provide fall protection to employees conducting cleaning work on the exterior portions of the windows at the Westlawn Construction site in Milwaukee, Wisconsin.  This exposed employees to fall hazards.

29 CFR 1910.1200(h): Employees were not provided effective information and training as specified in 29 CFR 1910.1200(h)(1) - (2) on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard was introduced in the workplace.    (a) Employees utilizing hazardous chemicals to clean a construction site were not provided with training on the hazards associated with the chemicals and the protective equipment to use when utilizing those chemicals.
