29 CFR 1926.405(g)(1)(i): Flexible cords and cables were not suitable for conditions of use and location: On or about July 17, 2012, the employer failed to ensure that flexible cords were suitable for use in wet locations. A flexible cord ran through water and was used as a power source for a DeWalt circular saw. Employees were using the circular saw to cut angle iron.

29 CFR 1926.405(g)(2)(iii): Splices. Flexible cords shall be used only in continuous lengths without splice or tap. Hard service flexible cords No. 12 or larger may be repaired if spliced so that the splice retains the insulation, outer sheath properties, and usage characteristics of the cord being spliced. On or about July 17, 2012, the employer failed to ensure that flexible cords were continuous in length without splicing. Employees were using a spliced flexible cord as a power source for a DeWalt circular saw.

Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. On or about July 17, 2012, the employer failed to ensure that flexible cords had adequate strain relief. The flexible cord was used by employee(s) as a power source for a DeWalt circular saw.
