29 CFR 1910.26(c)(3)(i): A simple rule for setting up a ladder at the proper angle is to place the base a distance from the vertical wall equal to one-fourth the working length of the ladder. On or about 7/23/2012 an employee working at the Besito Restaurant was exposed to approximately 10' falls. Employees ladder was not at the proper angle.
