29 CFR 1926.20(b)(2):     The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person .    a) On or about June 13 thru June 25, 2012 the employer did not ensure that designated competent person onsite performed a jobsite inspection and address the fall hazard potentials from a two story residential roof being torn off and replaced.

29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee who might be exposed to fall hazards:    a) On or about June 13 thru June 25, 2012, the employer did not ensure that each employee exposed to fall hazard potential while engaged in roofing operations on a residential house at a height of 20 feet and greater from the roof eave to the lower ground were first trained in the fall protection requirements to identify and correct that fall potential before work began.

29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.    a) On or about June 13 thru June 25, 2012 the employer did not provided conventional or person fall protection for the employees engaged in a re-roof project on a two story residential home that had a roof  pitch greater than 7 in 12 and at height great than 20 feet to the lower ground level.
