29 CFR 1926.1053(b)(1):  Where portable ladder was used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed:    This violation occurred on July 25, 2012, at a job site located at 6239 Lakeshore Drive, Dallas, Texas 75214; where at least (4) employees used an extension ladder that did not extend 3-ft above the upper landing nor was it secured at the top to keep the employees from falling to the ground below.
