29 CFR 1910.67(c)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. (a) Jobsite - On or about July 11, 2012 an employee was exposed to a 30 foot fall hazard when cutting down a sign from the platform of an aerial lift without wearing a body harness with lanyard attached to the boom or basket.

29 CFR 1910.135(a)(1): The employer shall ensure that each affected employee wears a protective helmet when working in areas where there is a potential for injury to the head from falling objects. (a) Jobsite - On or about July 11, 2012 employees were not wearing head protection while working underheath metal structures which had been removed form a pole.

29 CFR 1910.252(b)(2)(i)(B): Goggles or other suitable eye protection shall be used during all gas welding or oxygen cutting operations. Spectacles without side shields, with suitable filter lenses are permitted for use during gas welding operations on light work, for torch brazing or for inspection. (a) Jobsite - On or about July 11, 2012 an employee did not wear eye protection while using a cutting torch to remove a sign. .
