This violation was observed on or about July 24, 2012, when employees were exposed to an electrical hazard while using a Baker Brand Resaw that had two motors that were being worked on by maintenance without locking or tagging out the energy isolating device for the motors.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: The employer does not ensure that all flexible cords are connected to fittings so that tension is not transmitted to terminal screws. This violation was observed on or about July 24, 2012, when employees were using a table saw that was powered by a flexible cord that had the outer insulation pulled back from the connection allowing strain to be applied to the terminal screws. Cougar Pallet, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(g)(2)(iii), which was contained in OSHA inspection number 314001017citation number 02 item number 001 and was affirmed as a final order on 03/10/2010, with respect to a workplace located at 13417 Aldine Westfield Rd, Houston, TX 77039.
