29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 1926.502(k) developed and implemented: (a) G&B Roofing @ 4616 Apache Rd., Boulder, CO 80303: On 7/24/12, the employer did not ensure that fall protection systems were utilized by employees engaged in re-roofing activities while working at the unprotected edges of a on a single-family residence. Employees were exposed to a fall hazard in excess of 25 feet. G&B Roofing was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA Inspection 313413635, citation number 1, item number 1, issued on 11/24/09, with respect to a workplace located at 408 Fossil Drive, Johnston, CO 80534.

29 CFR 1926.1053(b)(1): The employer did not ensure that when portable ladders were used to access the upper landing of the building the ladder side rails were not extended at least 3ft above the upper landing surface: (a) G&B Roofing @ 4616 Apache Rd., Boulder, CO 80303: On 7/24/12, the employer did not ensure that employees utilized portable ladders in a way that enabled them to access and egress from the roof using ladders that extended at least 3 feet above the walking and working surface. Employees used a portable extension ladder for access to a roof of a single family home. Employees were exposed to a fall hazard of approximately 14 feet while attempting to use the ladder. G&B Roofing was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926.1053(b)(1) which was contained in OSHA Inspection 313413635, citation number 1, item number 2, issued on 11/24/09, with respect to a workplace located at 408 Fossil Drive, Johnston, CO 80534.
