29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: (a) On or about 7/25/12, in the production area, a personnel basket was used with forklifts without written approval of the manufacturer. Employees were exposed to falls of more than 4 feet to the concrete below.
