29 CFR 1910.212(a)(2):     Machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points and rotating parts:    (a) On June 27th, 2012, employees of Ozark Machine Co. were exposed to caught in hazards from exposed rotating parts on at least four different drill presses throughout the facility.

29 CFR 1910.215(a)(4)     Work rests on abrasive grinders were either not installed or not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest.     (a) On June 27th, 2012, employees of Ozark Machine Co. were exposed to caught in and struck by hazards from an abrasive grinding wheel on the workroom floor that did not have an appropriately adjusted work rest. The work rest opening was approximately three quarters of an inch to one inch in width.

29 CFR 1910.305(g)(2)(iii)    Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws in areas listed but not limited to:    (a) On June 27th, 2012, employees of Ozark Machine Co. were exposed to contact with electrical current hazards due to exposed wiring on the conveyor of Machine 2001 in the milling department. Adequate strain relief was not provided causing the fitting of the wire insulation to break loose from the electrical box on the machine.

29 CFR 1910.334(a)(2)(ii):     There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service:    (a) On June 27th, 2012, employees of Ozark Machine Co. were exposed to contact with electrical current hazards when a Dremmel 100 L rotary tool was used while wiring was exposed near the area that employees hold the tool.
