29 CFR 1910.305(g)(2)(iii):  Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:    (a)  Soda Springs Ranger District Warehouse, on or about July 18, 2012, and at times prior thereto, strain relief for flexible cord to the Old Paint Shaker was pulled loose creating a potential electrical shock hazard.    Note:  Abatement certification is required for this item.

29 CFR 1910.110(b)(6)(vi):  Readily ignitable material such as weeds and long dry grass was not removed within 10 feet of any container.    (a)  Soda Springs Ranger District Warehouse, on or about July 18, 2012, and at times prior thereto,  1000 gallon propane storage tank had grass and weeds growing immediately behind the tank.    NOTE:  U.S. Forest Service was previously cited REPEAT for a violation of this occupational safety and health standard, which was contained in OSHA inspection number Inspection #92793, Notice 1  Item 2 ; and was affirmed by an Abatement Certification signed and dated October 28, 2011, with respect to a workplace located at U.S. Forest Service, McCall Ranger District; 102 W. Lake Street  Mccall ID 83638.     NOTE:  U.S. Forest Service was previously cited REPEAT for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 309969889, citation number 2, item number 4, and was affirmed by an Abatement Certification signed and dated May 12, 2008, with respect to a workplace located at Challis Middle Fork, Challis, ID  83226.      NOTE:  U.S. Forest Service was previously cited SERIOUS for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 309972248, citation number 2, item number 4, and was affirmed by an Informal Settlement Agreement signed and dated December 12, 2008, with respect to a workplace located at Nez Perce NF, Salmon River Road, White Bird, ID. 29 CFR 1910.213(h)(1):  The sides of the lower exposed portion of the blade of radial saw(s) were not guarded to the full diameter of the blade by a device that automatically adjusted itself to the thickness of the stock and remained in contact with the material:    (a) Soda Springs Ranger District Warehouse; on or about July 18, 2012, and at times prior thereto, DeWalt 540 Radial arm saw; SN 361688 did not have a guard on the lower half of the blade.      NOTE:  U.S. Forest Service was previously cited REPEAT for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 92793, citation number 3, item number 4, and was affirmed by an Informal Settlement Agreement signed and dated May 24, 2011, with respect to a workplace located at McCall Ranger District, 102 W.
