29 CFR 1910.304(g)(5):     Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective.    A)  At the workplace, in the Set Packing Area, on line B1, an electrical fan did not have a continuous path to ground due to a missing grounding pin.    B)  At the workplace, in the Set Packing Area, on line C1, an electrical fan did not have a continuous path to ground due to a missing grounding pin.    C)  At the workplace, in the Set Packing Area, electrical label machine number 1 did not have a continuous path to ground due to a missing grounding pin.    D)  At the workplace, in the Set Packing Area, electrical label machine number 4 did not have a continuous path to ground due to a missing grounding pin.    E)  At the workplace, in the Set Packing Area, electrical label machine number 6 did not have a continuous path to ground due to a missing grounding pin. 29 CFR 1910.305(b)(1)(ii):     Unused openings in cabinets, boxes, and fittings shall be effectively closed.    A)  At the workplace, in the Set Packing Area, on C1 Line, a junction box attached to an industrial fan had an approximately one inch opening exposing open wiring that was not effectively closed with a knockout plug.

29 CFR 1910.305(b)(2)(i):     All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.    A)  At the workplace, near lines F1 and F2 of the Set Pack Area, there was an electrical outlet mounted below the conveyor belt that did not have a properly installed faceplate to prevent employee exposure to contact with electrical current from exposed open wiring.

A)  At the workplace, in the Set Pack Area, employees were using UPC labeling machine # 1 that was made in house by the employer which utilized a flexible cord.    B)  At the workplace, in the Set Pack Area, employees were using UPC labeling machine # 4 that was made in house by the employer which utilized a flexible cord.  The flexible cord was installed in the UPC label machine with a knot which created a fire hazard.    C)  At the workplace, in the Set Pack Area, employees were using UPC labeling machine # 6 that was made in house by the employer which utilized a flexible cord.      D)  At the workplace, in the Set Pack Area, employees were using UPC labeling machine # 12 that was made in house by the employer which utilized a flexible cord.     E)  At the workplace, in the Set Pack Area, employees were using UPC labeling machine # 13 that was made in house by the employer which utilized a flexible cord.      F)  At the workplace, in the Set Pack Area, employees were using UPC labeling machine # 18 that was made in house by the employer which utilized a flexible cord. B)  At the workplace, in the Set Packing Area,  on the C2 line, a black flexible cord rated for 220 VAC was used as a substitute for the fixed wiring of fans and conveyors.

A)  At the workplace, in the Set Packing Area, a retractable flexible cord mounted on the ceiling was not connected to a device and fitting so that strain relief was provided to the female end connector.    B)  At the workplace, in the Set Packing Area, a flexible cord connected to an industrial fan was not connected to a device and fitting so that strain relief was provided.

A)  At the workplace, in the Set Pack Area, on the C1 line, a four prong 220 volt outlet box had electrical tape holding the cover in place exposing employees to electrical hazards.
