29 CFR 1926.102(a)(1):  Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    a)  R S Facia and Sheathing at 2950 and 2960 Uinta Street, Denver, CO 80238:  On or before July 21, 2012, the employer did not ensure that employees wore safety glasses when using or working in the vicinity of pneumatic nail guns.  Condition exposed employees to the hazard of projectiles to the eyes.

29 CFR 1926.501(b)(13):  Employees engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems, nor were employees provided with an alternative fall protection measure under another provision of paragraph (b).  Note:  If using these systems was infeasible or created a greater hazard, the employer did not develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502:    a)  R S Facia and Sheathing at 2950 and 2960 Uinta Street, Denver, CO 80238:  On or before July 21, 2012, the employer did not ensure that employees were using fall protection in the following instances:    1)  Two employees were sheathing the roof, with a pitch af about 7 in 12, without the use of personal fall arrest systems.  Condition exposed employees to fall hazards of up to 21';    2)  The window openings on the second floor included four 36" wide windows with the window ledges 24" above the floor, and three 24" wide windows with the window ledges 36" above the floor.  These widow openings did not have upper guardrails.  Condition exposed employees to fall hazards of up to 12'6";     3)  The stairway opening on the second floor did not have any guardrails around the landing, measuring 13' long and 40" wide, with a fall hazard of 9'; and    4)  Each unit had one floor opening measuring 24" x 32".  Condition exposed employees to fall hazards of 5' from the main floor to the crawl space.     Abatement Note:  29 CFR 1926.502(b) provides that the top edge of the top gaurdrail shall be 42 inches +/- 3", and the midrail shall be at a height midway between the top rail and the walking/working surface.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches, whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge:    a)  R S Facia and Sheathing at 2950 and 2960 Uinta Street, Denver, CO 80238:  On or before July 21, 2012, the employer did not ensure that the stairway to the second floor included stairrails on the open side.  The stairs included 15 steps with a 7 1/2'" rise over an 11" run, were 36" wide, and were open on one side.  Condition exposed employees to a 9' fall hazard.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:    a)  R S Facia and Sheathing at 2950 and 2960 Uinta Street, Denver, CO 80238:  On or before July 21, 2012, the employer provided a red and black extension cord with damaged outer insulation for use in powering a 110 volt Skilsaw, model HD 77, sn HH109759.  Condition exposed employees to electrical shock hazards.
