(a) Mon Valley Health Clinic, Fairmont, WV: A wooden scaffold was dismantled by an employee on the side of a building under construction without the direction of a competent person to identify an alternate means of fall protection that would protect the employee on the scaffold from an approximate thirteen foot fall as documented on July 24, 2012. ABATEMENT CERTIFICATION REQUIRED.
