29 CFR 1926.20(b)(2):  The employer did not develop an accident prevention program, that provided for frequent and regular inspections of the job sites, materials, and equipment, to be made by competent persons designated by the employer.      (a) Pine Hill Middle School, 1100 Turnersville Road, Pine Hill NJ: The employer failed to conduct frequent and regular inspections of the jobsite. Subcontractors of D.D.S. Services LLC were working at heights ranging from 24 to 32 feet above ground without the use of fall protection, misusing a metal extension ladder, allowing the subcontractors to use  damaged flexible cords and connecting them as to reverse polarity, not wearing hard hats where there was exposure to falling debris and not wearing safety glasses where there was a potential for flying particles, occurred on or about 7/26/12.    NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury:    (a) Pine Hill Middle School, 1100 Turnersville Road, Pine Hill NJ: The employer failed to train the employee in the recognition and avoidance of unsafe conditions and practices located at their worksite. 29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.      (a) Jobsite located at Pine Hill Middle School, 1100 Turnerville Road, Pine Hill, NJ: The employee of D.D.S. Services LLC and employees of JC Construction were performing roofing activities, at heights ranging from 24 to 34 feet above ground, on a low sloped roof approximatley 68 feet in width, were not protected from falling, observed on or about 7/26/12.    NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
