(a) Pine Hill Middle School, 1100 Turnersville Road, Pine Hill NJ: The employer failed to conduct frequent and regular inspections of the jobsite. Subcontractors of D.D.S. Services LLC were working at heights ranging from 24 to 32 feet above ground without the use of fall protection, misusing a metal extension ladder, allowing the subcontractors to use  damaged flexible cords and connecting them as to reverse polarity, not wearing hard hats where there was exposure to falling debris and not wearing safety glasses where there was a potential for flying particles, occurred on or about 7/26/12.    NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
