29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet(1.8m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system, or a combination of a warning line system and guardrail system, warning line system and safety net system, warning line system and personal fall arrest system, or warning line system and safety monitoring system; or, on roofs 50 feet (15.25m) or less in width, the use of a safety monitoring system alone: (a) East side of Building (Roof) located at 4040 Bankhead Highway, Douglasville, GA 30133: Employees engaged in roofing activities on a low-slope roof were not provided with a fall protection system on a roof at heights of 12' to 16' along the side edge of the roof, exposing them to fall hazards.

29 CFR 1926.1053(b)(16): Portable ladder(s) with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identified them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired: (a) East side of building (Roof Access) located at 4040 Bankhead Highway, Douglasville, GA 30133: The Werner 32' Aluminum Ladder used to access the roof had a damaged lower rung and was not removed from service, exposing employees to fall hazards.
