29 CFR 1926.95(a):     Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:      (a) On or about 07/26/2012 - 5100 Highgway 31 South, Calera, AL, employees mixing and working with Portland Cement were not wearing gloves.

29 CFR 1926.451(e)(1):     When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:        (a) On or about 07/26/2012 - 5100 Highgway 31 South, Calera, AL, employees used the crossbraces to access the scaffolding system.

29 CFR 1926.451(g)(1):     Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level:        (a) On or about 07/26/2012 - 5100 Highgway 31 South, Calera, AL, employees working from the fabricated scaffolding were not protected from falling approximately 14 feet to the ground.
