29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.      On or about 7/25/2012 at 1235 Druid Park Ave, Augusta, GA.: An employee was observed climbing the end frames of a walk-thru scaffold to access the working level. The employee was exposed to a fall of more than 14 feet to the lower level.

29 CFR 1926.451(g)(1):  Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.        On or about 07/25/2012 Druid Park Ave, Augusta, GA: An employee, working on the second buck of a walk-through scaffold, was exposed to a fall of more than 10 feet to the lower level, when receiving mud on the upper working level of the scaffold. There were no guard rails nor was the employee wearing fall protection at the time.

29 CFR 1926.102(a)(4): Face and eye protection was not kept clean and in good repair, and/or the use of defective protective equipment was not prohibited:        On or about 07/25/2012 at Druid Park Ave, Augusta, GA: An employee cutting block was exposed to a struck by hazard from flying debris while cutting concrete blocks. The face shield he was wearing was dirty and the employee could not see his work clearly. To see the work clearly, he pushed the face shield up and out of the way while he making cuts on the concrete blocks.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible: (Construction Reference 1926.103)        On or about 07/25/2012 at Druid Park Ave, Augusta, GA: Employees using N95 type respirators while mixing mortar containing crystilline slicia or while cutting concrete block were exposed to respiratory tract irritation.  The employer did not provide them with the information in Appendix D to 29 CFR 1910.134.
