Marine Corps Base Quantico 615 SNCO Academic Facility: On or about 07/25/12, the extension cords and outlet adapter used to supply power to the Dewalt band saw and Bosch drill was not used in accordance with its listing and labeling in that the orange three way adapter was not plugged into a receptacle outlet.
