29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Sam Hostetler dba Hostetler Roofing, Findlay, Ohio: On or about July 26, 2012, at address 826 S. Blanchard St., Findlay, Ohio, the employer did not ensure that employees were protected from falls while they were engaged in roofing work on a steep sloped roof (10:12) on a residential type structure. Employees were exposed to a potential fall of approximately 18.7 feet to the ground below, which could result in permanent disability for death.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) Sam Hostetler dba Hostetler Roofing, Findlay, Ohio: On or about July 26, 2012, at address 826 S. Blanchard St., Findlay, Ohio, the employer did not ensure that employees had been trained in the appropriate use of fall protection, such as but not limited to the use of personal fall arrest systems. Employees were exposed to falls of approximately 18.7 feet to the ground below, which could result in permanent disability or death.
