On or about July 26, 2012 and times prior thereto employees were exposed to tripping hazards from the accmulations of scrap and debris on the step leading to the operator's platform near the edging station.

29 CFR 1910.24(b):     Fixed stairs were not provided for access from one structure level to another where operations necessitated regular travel between levels, and for access to operating platforms at any equipment which requires attention routinely during operations:    On or about July 26, 2012 and times prior thereto, employees were exposed to trip/fall hazards accessing the work platform located near the edging station.  The platform is approximately 33 inches above the ground and loose lumber is being used in lieu of a fixed step.

Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets. 29 CFR 1910.305(b)(2)(i):     All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.    On or about July 26, 2012 and times prior thereto, employees were exposed to shock and fire hazards created by missing covers on electrical boxes in the following instances:    instance a.) on the junction box for the main air compressor for the mill.  instance b.) on the hydraulic unit for the side transfer conveyor off of the head rig.

29 CFR 1910.305(g)(1)(iv)(B):     Flexible cords and cables were run through holes in walls, ceilings, or floors    On or about July 26, 2012 and times prior thereto, employees were exposed to shock and fire hazards from a flexible electrical power cord for the hydraulic unit at the side transfer conveyor off of the head rig.  The flexible power cord was run through a hole in the wall at the electrical room and run along the corrugated metal building wall to the unit.
