29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Callies Performance Products, Inc. - Fostoria, Ohio: On or about August 2, 2012, the employer did not assure that machine specific energy control procedures were developed, documented and utilized to control hazardous energy when employees entered the GFM milling machine to perform set-up activities. Employees were exposed to crushing and rotating part hazards in in that energy control was not utilized before employees entered hazard areas to clean, grease and set-up the machine. b) Callies Performance Products, Inc. - Fostoria, Ohio: On or about May 24, 2012, the employer did not assure that machine specific energy control procedures were developed, documented and utilized to control hazardous energy when employees serviced the BR678 grinder. Employees were exposed to the unexpected start-up of the machine when changing the wheel during a time in which the machine was not locked out. c) Callies Performance Products, Inc. - Fostoria, Ohio: On or about July 19, 2012, the employer did not assure that machine specific energy control procedures were developed, documented and utilized to control hazardous energy when employees conducted set-up activities on the CellCon horizontal CNC machine center (#2130 and #2126). Employees were exposed to the unexpected start-up of the machine when adjusting and/or changing components during a time in which the machine was not locked out. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices: a) Callies Performance Products, Inc. - Fostoria, Ohio: On or about May 24, 2012, the employer did not assure that employees who service machines such as, but not limited to, grinders and horizontal milling machines, were trained in the skills necessary for the safe application and use of lockout and energy isolating devices to protect against unexpected start-up. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Callies Performance Products, Inc. - Fostoria, Ohio: On or about August 2, 2012, employees operating the GFM milling machine #2002 were exposed to injury associated with the machines pinch point as the cutting head side of the machine closed. The employer did not assure the pinch point was guarded to protect employees operating the machine. b) Callies Performance Products, Inc. - Fostoria, Ohio: On or about August 2, 2012, employees operating machine #2003 were exposed to injuries associated with the machines unguarded rotating parts. c) Callies Performance Products, Inc. - Fostoria, Ohio: On or about July 27, 2012, the employer did not assure that polisher #2033 was guarded to protect employees from the rotating parts hazard. d) Callies Performance Products, Inc. - Fostoria, Ohio: On or about June 8, 2012, employees operating the Cellcon machine (# 2130&2126) were exposed to injury associated with the machines rotating parts and pinch points. The employer did not assure that the machine was adequately guarded to protect the operator from moving parts. e) Callies Performance Products, Inc. - Fostoria, Ohio: On or about July 25, 2012, employees that operated the Mazak horizontal milling machine #2028 were exposed to pinch points and rotating parts. The employer did not assure the machine was guarded to protect the operator from accessing the hazard area during a time in which the machine could operate. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
