29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection. a) On or about 7/26/12 at the Dunkirk Middle School rehabilitation project located at 525 Eagle Street, Dunkirk, New York; employees working in the corridors were using grinders and vacuums to refinish a terrazzo floor. The vaccum was plugged into an extension cord and there was no ground fault circuit interrupter in use. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.405(b)(1): Unused openings in cabinets, boxes, and fittings were not effectively closed: a) On or about 7/26/12 at the Dunkirk Middle School rehabilitation project located at 525 Eagle Street, Dunkirk, New York; employees working in the corridors were using floor grinder to refinish a terrazzo floor. The circuit panel box mounted on the side of the grinder was missing a knock out. NO ABATEMENT CERTIFICATION REQUIRED
