29 CFR 1926.20(b)(2):     Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.    An Employee working from a five in twelve pitch roof was exposed to falls of approximately 21 feet to a lower level. The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person.

29 CFR 1926.307(d)(1):     "Guarding." Pulleys, any parts of which are 7 feet (2.128 m) or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than 6 ft. 6 in. (1.976 m) from the floor or platform may be guarded with a disk covering the spokes.    Employees are exposed to nip points while reaching over the unguarded portion of the pulley and belts turning on and off the air compressor.

29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)  Note:  If using these systems was infeasible or created a greater hazard, the employer should develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502.      Employees were engaged in roofing activities working from a 5 in 12 pitch roof, employees were exposed to falls of approximately 21 feet from the ground to the eave. Employees were not protected from falling by guardrail systems, safety net systems, or a personal fall arrest systems.
